You purchased a test kit online from 111harleystreet.com for pick-up at the 111 Harley St Clinic.
You were instructed to register your test kit online.

You can find the DNA Test Kit ID on the coin envelope inside your test kit package.

While the clinic is required to know our patients and document their interactions, please be assured that your specimen will be de-identified when sent to the lab for DNA extraction. This means the lab will never know your name.

Neither the lab nor the clinic sells your data to third parties.
